Ghost Inventory: The Stock the System Swears Is There

Your WMS says you have 48 units of a SKU. Your picker walks the aisle, scans the location, and finds cardboard and air. So you short the order, the customer complains, and somewhere in a morning standup someone says "we need to fix our inventory accuracy" like that's a new idea.
Ghost inventory is stock the system is certain exists but doesn't. It builds up quietly. A receiving team that scans a pallet at the dock but never confirms putaway. A return that gets processed in the system but lands in a staging lane and never makes it back to a slot. A transfer between locations that completes on paper when someone hits "confirm" too early. Over six months those small gaps compound, and suddenly you're looking at a 94% inventory accuracy rate wondering why it feels worse than that number sounds. It does, because the ghosts tend to cluster in your fastest-moving SKUs.
The instinct is to schedule a big wall-to-wall count and zero everything out. That works for about three weeks. Then the ghost inventory starts rebuilding, because the count fixed the data without fixing the process that corrupted it in the first place.
What actually helps is counting the locations you're most suspicious of, frequently, before you have a miss. If a location gets touched every day, it should get counted more than once a quarter. And you need a system that makes that fast enough that a picker can do a quick recount during a slow moment rather than waiting for a dedicated count team on a Saturday.
The goal isn't a perfect number on a dashboard. It's fewer moments where a picker walks an aisle and comes back empty-handed 😑 while the system is still convinced the product is there.
